Patient Rights & Provider Responsibilities


At Courageous Counseling & Consulting, we believe every client deserves transparency, respect, and clear information about their rights under Texas law. As required by Texas House Bill 4224 and the Texas Behavioral Health Executive Council (BHEC), this page provides detailed instructions on how to:

  • Request your health care records
  • Contact the Texas Behavioral Health Executive Council
  • File a consumer complaint with the Texas Office of the Attorney General

Requesting Your Health Care Records

You have the legal right to request a copy of your health care records at any time. To request your records:

  1. Submit a written request via email to: info@courageouscounselingconsulting.com
  2. Include your full name, date of birth, and the records you are requesting
  3. Requests will be processed in accordance with Texas state law

This agency oversees professional counselors, marriage and family therapists, social workers, and psychologists in the state of Texas.

Filing a Consumer Complaint with the Attorney General

Texas HB 4224 requires all mental health providers to clearly inform clients how to file a complaint with the Office of the Attorney General.

The Attorney General’s office investigates consumer fraud, deceptive practices, and other violations that may affect health care consumers in Texas.

Additional Rights as a Counseling Client

As a client of Courageous Counseling & Consulting, you also have the right to:

  • Receive counseling services free from discrimination
  • Participate in developing your treatment plan
  • Decline any service or referral offered
  • Understand the nature and purpose of counseling approaches used
  • Ask questions about your care at any time
  • Confidentiality consistent with Texas law and professional ethics
